Hepatitis B is a viral infection that affects the liver and can cause both acute and chronic disease. The virus is transmitted through contact with infected blood or other body fluids. Laboratory testing plays a crucial role in diagnosing hepatitis B infection, monitoring disease progression, and determining the need for treatment. Understanding the various hepatitis B tests available can help patients and healthcare providers make informed decisions about screening and management.
The hepatitis B surface antigen test, known as HBsAg, is the primary screening test used to detect active hepatitis B infection. When this test is positive, it indicates that the person is currently infected with the virus and can potentially transmit it to others. The test detects a protein on the surface of the hepatitis B virus that appears in the blood during both acute and chronic infections. If HBsAg remains positive for more than six months, it suggests that the infection has become chronic. Additional tests are often performed alongside HBsAg to provide a complete picture of the infection status.
The hepatitis B surface antibody test, or anti-HBs, measures antibodies that the immune system produces in response to the hepatitis B surface antigen. A positive anti-HBs test typically indicates immunity to hepatitis B, either from previous infection that has resolved or from successful vaccination. This test is commonly used to confirm that vaccination has been effective and to determine if a person is protected against future hepatitis B infection. Healthcare workers and others at high risk for exposure often have their anti-HBs levels checked to ensure adequate immunity.
The hepatitis B core antibody test, abbreviated as anti-HBc, detects antibodies against the core protein of the hepatitis B virus. This test comes in two forms: IgM anti-HBc, which indicates recent or acute infection, and total anti-HBc, which can be positive in both acute and chronic infections as well as in resolved past infections. The presence of anti-HBc helps distinguish between different stages of hepatitis B infection and can identify people who have been exposed to the virus at some point in their lives, even if they have cleared the infection.
For individuals diagnosed with chronic hepatitis B, additional laboratory tests are important for monitoring disease activity and liver health. These include hepatitis B viral load testing, which measures the amount of virus in the blood, and liver function tests that assess how well the liver is working. The hepatitis B e-antigen and its corresponding antibody tests provide information about viral replication and infectivity. Regular monitoring through these laboratory tests helps healthcare providers determine when treatment is necessary and assess the effectiveness of antiviral therapy in managing chronic hepatitis B infection.
