Lactate dehydrogenase, commonly known as LDH, is an enzyme found in nearly all body tissues, with particularly high concentrations in the heart, liver, muscles, kidneys, lungs, and blood cells. When cells are damaged or destroyed, LDH is released into the bloodstream, making it a useful marker for detecting tissue damage. A blood test measuring LDH levels helps healthcare providers identify and monitor various conditions involving cell or tissue injury.

The LDH test is typically ordered when a person shows symptoms suggesting tissue damage or when monitoring known conditions. Common reasons for testing include unexplained fatigue, muscle weakness, chest pain, abdominal pain, or signs of anemia. The test is also valuable in diagnosing and monitoring hemolytic anemia, where red blood cells are destroyed prematurely, and in evaluating certain cancers, particularly lymphomas and leukemias. Additionally, LDH levels can help assess the severity of conditions such as heart attacks, liver disease, and muscle disorders.

Normal LDH levels generally range from approximately 140 to 280 units per liter, though reference ranges may vary slightly between laboratories. Elevated LDH levels indicate tissue damage somewhere in the body, but because the enzyme is present in so many tissues, an increased level alone cannot pinpoint the exact location or cause of damage. Very high levels may suggest conditions such as severe anemia, extensive tissue injury, certain cancers, liver disease, muscle disorders, or lung problems. Moderately elevated levels might indicate milder tissue damage or chronic conditions.

When LDH levels are abnormal, additional tests are usually needed to determine the specific cause and location of tissue damage. These may include LDH isoenzyme tests, which identify which of the five types of LDH is elevated, helping to narrow down which organ or tissue is affected. Other complementary tests might include liver function tests, muscle enzyme tests, complete blood counts, or imaging studies. The LDH test is most valuable when interpreted alongside other laboratory results and clinical findings, providing important information for diagnosis and monitoring treatment effectiveness.
