Liver function tests are a group of blood tests that measure different enzymes, proteins, and substances produced or processed by the liver. These tests help doctors evaluate how well the liver is working and can detect liver damage, inflammation, or disease. The most common liver function tests include measurements of alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), albumin, bilirubin, and total protein. Each of these markers provides specific information about different aspects of liver health and function.
ALT and AST are enzymes found primarily in liver cells. When liver cells are damaged or destroyed, these enzymes leak into the bloodstream, causing their levels to rise. Elevated ALT is often more specific to liver damage, while AST can also be elevated in heart or muscle problems. Alkaline phosphatase is an enzyme related to the bile ducts, and elevated levels may indicate bile duct obstruction or bone disorders. Bilirubin is a yellow pigment produced when red blood cells break down, and the liver normally processes and eliminates it. High bilirubin levels can cause jaundice and may indicate liver dysfunction or bile duct problems.
Albumin and total protein measurements assess the liver's ability to produce proteins necessary for various body functions. Albumin is the main protein made by the liver and helps maintain fluid balance in the blood vessels. Low albumin levels can indicate chronic liver disease or other conditions affecting protein production. The prothrombin time test, sometimes included in liver function panels, measures how long it takes blood to clot and reflects the liver's production of clotting factors.
Preparation for liver function tests typically requires fasting for 8 to 12 hours before the blood draw, though some tests may not require fasting. Certain medications, supplements, and alcohol consumption can affect test results, so patients should inform their healthcare provider about all substances they are taking. The blood sample is usually collected from a vein in the arm, and results are typically available within a few days. Abnormal results may require additional testing or imaging studies to determine the underlying cause and appropriate treatment.
