Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is a crucial laboratory test used to monitor long-term blood sugar control in people with diabetes. This test measures the percentage of hemoglobin proteins in red blood cells that have glucose attached to them. Since red blood cells typically live for about three months, the HbA1c test provides an average picture of blood glucose levels over the past two to three months, making it more comprehensive than daily blood sugar measurements that only show glucose levels at a single point in time.

The HbA1c test is performed using a simple blood sample, which can be drawn at any time of day without requiring fasting. When glucose circulates in the bloodstream, it naturally binds to hemoglobin in a process called glycation. The higher the blood glucose levels over time, the more glucose becomes attached to hemoglobin. For people without diabetes, normal HbA1c levels are typically below 5.7 percent. A level between 5.7 and 6.4 percent indicates prediabetes, suggesting an increased risk of developing type 2 diabetes. An HbA1c level of 6.5 percent or higher on two separate tests is one of the criteria used to diagnose diabetes.

For individuals already diagnosed with diabetes, the HbA1c test serves as an essential tool for monitoring how well their treatment plan is working. Most diabetes management guidelines recommend that people with diabetes aim for an HbA1c level below 7 percent, though individual targets may vary based on age, overall health, and other factors. Healthcare providers typically recommend having this test done at least twice a year if diabetes is well controlled, or quarterly if treatment has recently changed or blood sugar targets are not being met.

It is important to note that certain conditions can affect the accuracy of HbA1c results. People with certain types of anemia, recent blood loss, blood transfusions, or certain hemoglobin variants may receive inaccurate results. In such cases, alternative tests like fructosamine or continuous glucose monitoring may be recommended. Despite these limitations, the HbA1c test remains the gold standard for assessing long-term glucose control and is an invaluable tool in preventing diabetes-related complications such as kidney disease, nerve damage, and cardiovascular problems.
