Hemoglobin A1c, commonly referred to as HbA1c or A1c, is a blood test that measures the average blood sugar levels over the past two to three months. This test is essential for diagnosing and monitoring diabetes. Unlike daily blood glucose tests that provide a snapshot of sugar levels at a specific moment, the A1c test gives a broader picture of how well blood sugar has been controlled over time. The test measures the percentage of hemoglobin proteins in red blood cells that have glucose attached to them. Since red blood cells live for about three months, the test reflects the average glucose exposure during that period.

For people without diabetes, a normal A1c level is below 5.7 percent. A level between 5.7 and 6.4 percent indicates prediabetes, which means blood sugar levels are higher than normal but not yet high enough to be classified as diabetes. An A1c level of 6.5 percent or higher on two separate tests indicates diabetes. For those already diagnosed with diabetes, the A1c test helps determine how well treatment plans are working. Most diabetes treatment guidelines recommend an A1c target of less than 7 percent for many adults with diabetes, though individual targets may vary based on age, overall health, and other factors.

The A1c test offers several advantages over regular blood glucose testing. It does not require fasting, so it can be done at any time of day regardless of when you last ate. The test is also less affected by short-term factors like stress, illness, or recent meals that can cause temporary spikes or drops in blood sugar. Healthcare providers typically recommend A1c testing at least twice a year for people with diabetes who are meeting treatment goals, and more frequently for those whose treatment has changed or who are not meeting their targets.

Certain conditions can affect the accuracy of A1c results. People with certain types of anemia, kidney disease, or those who have recently received blood transfusions may get inaccurate results. Additionally, some hemoglobin variants, which are more common in certain ethnic groups, can interfere with test results. If there are concerns about accuracy, healthcare providers may use alternative tests or different A1c testing methods. Understanding your A1c level is an important part of managing diabetes and preventing complications such as heart disease, kidney damage, nerve problems, and vision loss.
