Thyroid function tests are essential laboratory examinations used to evaluate how well your thyroid gland is working. The thyroid is a small butterfly-shaped gland located in the front of your neck that produces hormones regulating your metabolism, energy levels, and overall body functions. When the thyroid produces too much or too little hormone, it can cause various health problems affecting your weight, mood, heart rate, and energy levels.

The most common thyroid function tests measure thyroid-stimulating hormone (TSH), free thyroxine (free T4), and free triiodothyronine (free T3). TSH is produced by the pituitary gland and signals the thyroid to produce hormones. When thyroid hormone levels are low, TSH levels rise to stimulate more production, and when thyroid hormone levels are high, TSH levels drop. Free T4 and free T3 are the active thyroid hormones that circulate in your blood and are available for your body to use. These tests together provide a comprehensive picture of thyroid function.

Your doctor may order thyroid function tests if you have symptoms suggesting thyroid problems, such as unexplained weight changes, fatigue, irregular heartbeat, anxiety, tremors, difficulty sleeping, hair loss, or changes in menstrual periods. These tests are also used to monitor people already diagnosed with thyroid disorders who are receiving treatment, to screen newborns for congenital hypothyroidism, and to evaluate thyroid nodules or an enlarged thyroid gland.

The thyroid function test requires a simple blood sample drawn from a vein in your arm. No special preparation is usually needed, though you should inform your doctor about any medications or supplements you take, as some can affect test results. Your doctor will interpret the results based on the specific combination of TSH, T4, and T3 levels. High TSH with low T4 typically indicates hypothyroidism, while low TSH with high T4 suggests hyperthyroidism. Normal ranges can vary slightly between laboratories, so results should always be discussed with your healthcare provider for proper interpretation and any necessary follow-up testing or treatment.
