Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is a crucial blood test used to diagnose and monitor diabetes. This test measures the percentage of hemoglobin proteins in red blood cells that have glucose attached to them. Since red blood cells live for approximately three months, the HbA1c test provides an average picture of blood sugar levels over the past two to three months. Unlike daily blood glucose tests that show immediate levels, HbA1c gives healthcare providers and patients a broader view of long-term glucose control.

The HbA1c test is particularly valuable because it does not require fasting and can be performed at any time of day. For diagnosis, an HbA1c level below 5.7 percent is considered normal, while levels between 5.7 and 6.4 percent indicate prediabetes, a condition where blood sugar is higher than normal but not yet in the diabetic range. An HbA1c level of 6.5 percent or higher on two separate tests indicates diabetes. For people already diagnosed with diabetes, the test is typically performed every three to six months to assess how well their treatment plan is working.

The target HbA1c level varies depending on individual circumstances. For most adults with diabetes, the general goal is to maintain an HbA1c below 7 percent, which corresponds to an average blood glucose of approximately 154 milligrams per deciliter. However, healthcare providers may recommend different targets based on factors such as age, duration of diabetes, presence of complications, and overall health status. Younger patients without complications might aim for stricter control, while older adults or those with other health conditions might have less stringent targets to avoid risks associated with low blood sugar.

Several factors can affect HbA1c results and should be considered when interpreting the test. Conditions that affect red blood cell turnover, such as anemia, recent blood loss, or hemoglobin variants, can lead to falsely high or low readings. Certain medications, kidney disease, and liver disease may also influence results. Additionally, people of African, Mediterranean, or Southeast Asian descent may have hemoglobin variants that can interfere with some testing methods, though most modern laboratories use methods that account for these variations.

Regular HbA1c testing is an essential tool in diabetes management because it helps prevent serious complications. Maintaining HbA1c levels within the recommended range significantly reduces the risk of diabetes-related complications including nerve damage, kidney disease, eye problems, and cardiovascular disease. The test results, combined with daily blood glucose monitoring and lifestyle modifications, enable individuals with diabetes and their healthcare teams to make informed decisions about treatment adjustments, dietary changes, and medication management to achieve optimal health outcomes.
