Laboratory tests play a crucial role in diagnosing and monitoring various health conditions, including those affecting the liver. One important test used to assess liver function is the measurement of alkaline phosphatase, commonly abbreviated as ALP. This enzyme is found throughout the body, particularly in high concentrations in the liver, bones, kidneys, and digestive system. When cells containing alkaline phosphatase are damaged or undergoing increased activity, elevated levels of this enzyme can appear in the bloodstream, making it a valuable marker for detecting certain medical conditions.

The alkaline phosphatase test is typically ordered as part of a comprehensive metabolic panel or liver function test panel. Healthcare providers may request this test when a patient exhibits symptoms suggesting liver disease, such as jaundice, abdominal pain, nausea, or fatigue. Additionally, the test can help monitor bone disorders, as ALP levels often rise when bone cells are actively forming new tissue. The test requires a simple blood sample, usually drawn from a vein in the arm, and patients may be asked to fast for several hours before the test to ensure accurate results.

Normal alkaline phosphatase levels vary depending on age, sex, and blood type. Children and adolescents typically have higher ALP levels than adults because their bones are still growing. Pregnant women also naturally have elevated levels due to ALP production in the placenta. In adults, normal ranges generally fall between 30 and 120 units per liter, though reference ranges may differ slightly between laboratories. Results outside the normal range do not necessarily indicate disease but warrant further investigation to determine the underlying cause.

Elevated alkaline phosphatase levels can indicate various conditions affecting the liver or bones. Liver-related causes include bile duct obstruction, hepatitis, cirrhosis, or liver cancer. Bone-related causes may include Paget disease, bone fractures, or bone cancer. Certain medications and vitamin D deficiency can also raise ALP levels. Conversely, low alkaline phosphatase levels are less common but may occur in conditions such as malnutrition, zinc deficiency, or certain genetic disorders. When ALP levels are abnormal, doctors often order additional tests to pinpoint the specific organ or tissue responsible for the elevation and determine appropriate treatment.
