Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is a crucial blood test that measures the average blood sugar levels over the past two to three months. This test is essential for diagnosing and monitoring diabetes. When sugar enters the bloodstream, it attaches to hemoglobin, the protein in red blood cells that carries oxygen throughout the body. The more sugar present in the blood, the more hemoglobin becomes glycated. Since red blood cells live for approximately three months, the HbA1c test provides a longer-term view of blood sugar control compared to daily glucose measurements.

The HbA1c test is expressed as a percentage or in millimoles per mole. For people without diabetes, normal HbA1c levels typically range below 5.7 percent. Levels between 5.7 and 6.4 percent indicate prediabetes, a condition where blood sugar is higher than normal but not yet high enough to be classified as diabetes. An HbA1c level of 6.5 percent or higher on two separate tests indicates diabetes. For individuals already diagnosed with diabetes, the general target is to maintain HbA1c levels below 7 percent, though specific goals may vary based on individual circumstances, age, and overall health status.

Healthcare providers use the HbA1c test for multiple purposes. It helps diagnose type 2 diabetes and prediabetes, and it is sometimes used alongside other tests to diagnose type 1 diabetes. For people already living with diabetes, regular HbA1c testing helps monitor how well their treatment plan is working. The test is typically recommended at least twice a year for diabetic patients who are meeting their treatment goals and have stable blood sugar control. Those who have recently changed their diabetes treatment or are not meeting their blood sugar targets may need testing more frequently, sometimes every three months.

One of the main advantages of the HbA1c test is that it does not require fasting and can be done at any time of day, making it more convenient than some other diabetes tests. However, certain conditions can affect the accuracy of HbA1c results. These include certain types of anemia, recent blood transfusions, chronic kidney or liver disease, and certain hemoglobin variants. Pregnancy, recent severe bleeding, and certain medications can also influence test results. People with these conditions may need alternative testing methods or more frequent monitoring to accurately assess their blood sugar control.
