Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is a crucial blood test used to measure average blood sugar levels over the past two to three months. This test is essential for diagnosing diabetes and prediabetes, as well as monitoring how well blood sugar is being controlled in people who already have diabetes. Unlike regular blood glucose tests that provide a snapshot of sugar levels at a single moment, the HbA1c test offers a comprehensive view of long-term glucose management. When glucose enters the bloodstream, it attaches to hemoglobin, the protein in red blood cells that carries oxygen. The more glucose in the blood, the more hemoglobin becomes glycated. Since red blood cells live for approximately three months, the HbA1c test reflects average blood sugar levels during that period.
The HbA1c test is typically recommended for adults over 45 years of age as part of routine health screening, and for younger individuals who are overweight or have risk factors for diabetes such as family history, high blood pressure, high cholesterol, physical inactivity, or polycystic ovary syndrome. For people already diagnosed with diabetes, regular HbA1c testing helps healthcare providers assess whether treatment plans are effective and whether adjustments to medication, diet, or lifestyle are needed. The test is particularly valuable because it does not require fasting and can be performed at any time of day, making it more convenient than traditional glucose tolerance tests.
HbA1c results are reported as a percentage, indicating what proportion of hemoglobin is glycated. Normal levels are generally below 5.7 percent. A result between 5.7 and 6.4 percent indicates prediabetes, meaning blood sugar levels are higher than normal but not yet high enough to be classified as diabetes. An HbA1c level of 6.5 percent or higher on two separate tests indicates diabetes. For people with diabetes, the treatment goal is usually to maintain HbA1c below 7 percent, though individual targets may vary based on age, overall health, and other factors. Lower HbA1c levels generally indicate better diabetes control and reduce the risk of complications such as kidney disease, nerve damage, eye problems, and cardiovascular disease.
Several factors can affect HbA1c results and should be considered when interpreting the test. Conditions that affect red blood cell turnover, such as anemia, recent blood loss, or blood transfusions, can lead to inaccurate readings. Certain hemoglobin variants, more common in people of African, Mediterranean, or Southeast Asian descent, may also interfere with some testing methods. Pregnancy, kidney failure, and liver disease can influence results as well. In such cases, healthcare providers may use alternative tests or additional monitoring methods to accurately assess blood sugar control. Regular monitoring through HbA1c testing, combined with home blood glucose monitoring and healthy lifestyle choices, forms the foundation of effective diabetes management and prevention of long-term complications.
