The test may also be recommended for individuals with a family history of blood clotting disorders or for pregnant women with a history of pregnancy complications related to clotting. Low antithrombin III levels can result from genetic mutations that are passed down through families, or they may develop due to various medical conditions. Acquired deficiency can occur in people with liver disease, since the liver produces this protein, or in those with severe infections, blood clots, or certain cancers. Other causes include taking certain medications like heparin, undergoing surgery, or experiencing significant blood loss. Pregnant women and those taking birth control pills may also have temporarily decreased levels.
High antithrombin III levels are less common and generally less clinically significant than low levels. Elevated levels may be seen in people taking certain anticoagulant medications, those with kidney disease, or individuals with vitamin K deficiency. In some cases, increased levels may occur after a blood clot has resolved or during the acute phase of hepatitis.
