Tumor markers are substances produced by cancer cells or by the body in response to cancer. These markers can be found in blood, urine, or other body fluids and tissues. Healthcare providers use tumor marker tests to help detect, diagnose, and monitor various types of cancer. While these tests are valuable tools, they are not perfect indicators of cancer on their own and must be interpreted alongside other clinical information and diagnostic tests.

Different types of cancer produce different tumor markers. Some common tumor markers include CA 125, which is associated with ovarian cancer; PSA (prostate-specific antigen) for prostate cancer; CEA (carcinoembryonic antigen) for colorectal and other cancers; CA 19-9 for pancreatic cancer; and AFP (alpha-fetoprotein) for liver cancer. However, elevated levels of these markers do not always mean cancer is present, as some benign conditions can also raise these values. Additionally, not all cancers produce detectable tumor markers, and some people with cancer may have normal marker levels.

Tumor marker tests serve several important purposes in cancer care. They can help screen for cancer in certain high-risk populations, assist in diagnosing cancer when combined with other tests, determine the extent or stage of cancer, predict how aggressive a cancer might be, monitor treatment effectiveness, and detect cancer recurrence after treatment. For example, if tumor marker levels decrease during treatment, it usually indicates the therapy is working. Conversely, rising levels may suggest the cancer is growing or has returned.

It is important to understand the limitations of tumor marker tests. These tests are rarely used alone for cancer screening or diagnosis because elevated levels can occur in non-cancerous conditions such as infections, inflammation, or benign tumors. False positives can cause unnecessary anxiety and lead to additional testing. False negatives are also possible, where marker levels remain normal despite cancer being present. Therefore, doctors typically use tumor marker tests in combination with imaging studies, biopsies, and physical examinations to make accurate diagnoses and treatment decisions.

If your doctor orders a tumor marker test, it will usually involve a simple blood draw, though some markers may be measured in urine or tissue samples. The frequency of testing depends on your specific situation, such as whether you are being monitored during treatment or after completing therapy. Always discuss your test results with your healthcare provider, who can explain what the numbers mean in the context of your overall health and medical history. Regular monitoring through tumor marker tests, when appropriate, can be an important part of managing cancer care and detecting problems early when they may be easier to treat.
