Hepatitis B is a viral infection that affects the liver and can lead to both acute and chronic disease. Understanding the various tests used to diagnose and monitor hepatitis B is essential for proper disease management. Laboratory testing plays a crucial role in identifying the infection, determining its stage, and guiding treatment decisions.

The hepatitis B surface antigen (HBsAg) test is typically the first screening test performed to detect active hepatitis B infection. When this antigen is present in the blood, it indicates that the person is currently infected with the virus and can potentially transmit it to others. If HBsAg remains positive for more than six months, the infection is considered chronic. The hepatitis B surface antibody (anti-HBs) test detects antibodies that develop either after successful vaccination or recovery from infection, indicating immunity to the virus. The hepatitis B core antibody (anti-HBc) test identifies antibodies against the core protein of the virus and helps distinguish between acute, chronic, and past infections.

Additional tests provide more detailed information about the infection status and viral activity. The hepatitis B e antigen (HBeAg) test indicates active viral replication and high infectivity when positive. Conversely, the hepatitis B e antibody (anti-HBe) suggests lower levels of viral replication. Hepatitis B viral DNA testing measures the actual amount of virus in the blood and is essential for monitoring disease progression and treatment response. This quantitative test helps doctors determine when to start antiviral therapy and assess how well treatment is working.

Liver function tests complement hepatitis B-specific tests by evaluating how well the liver is working. These include tests for alanine aminotransferase (ALT) and aspartate aminotransferase (AST), which are enzymes that become elevated when liver cells are damaged. Bilirubin and albumin levels, along with prothrombin time, provide additional information about liver function. In some cases, doctors may also recommend a liver biopsy or non-invasive imaging tests to assess the degree of liver damage or fibrosis.

Regular monitoring through laboratory testing is important for people with chronic hepatitis B infection. Testing schedules vary depending on whether the person is receiving treatment, the stage of their disease, and their liver function status. Early detection and appropriate monitoring through these laboratory tests enable healthcare providers to make informed decisions about treatment timing and help prevent serious complications such as cirrhosis and liver cancer.
