Liver function tests are a group of blood tests used to evaluate how well the liver is working and to detect liver damage or disease. These tests measure the levels of various enzymes, proteins, and substances produced or processed by the liver. Common components of liver function tests include alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), albumin, bilirubin, and total protein. Elevated or decreased levels of these markers can indicate different types of liver problems, ranging from mild inflammation to serious conditions like hepatitis, cirrhosis, or liver failure.

ALT and AST are enzymes found primarily in liver cells. When liver cells are damaged or destroyed, these enzymes leak into the bloodstream, causing their levels to rise. ALT is more specific to the liver, while AST can also be elevated in heart or muscle damage. Alkaline phosphatase is an enzyme related to the bile ducts, and elevated levels may suggest bile duct obstruction or bone disorders. Bilirubin is a waste product from the breakdown of red blood cells that the liver processes and excretes. High bilirubin levels can cause jaundice, a yellowing of the skin and eyes, and may indicate liver dysfunction or bile duct problems.

Albumin and total protein measurements assess the liver's ability to produce essential proteins. Albumin is the main protein made by the liver and helps maintain fluid balance in the body. Low albumin levels can indicate chronic liver disease or malnutrition. The total protein test measures all proteins in the blood, including albumin and globulins. Abnormal results may suggest liver disease, kidney disease, or other health conditions affecting protein production or loss.

Liver function tests are commonly ordered for several reasons. They may be used to screen for liver infections such as hepatitis, monitor the progression of known liver disease, check for side effects of medications that can affect the liver, or evaluate symptoms like jaundice, abdominal pain, or unexplained fatigue. These tests are also important for people with risk factors for liver disease, including excessive alcohol consumption, obesity, diabetes, or a family history of liver problems. The tests require a simple blood sample, usually taken from a vein in the arm, and typically do not require fasting, though your healthcare provider may give specific instructions.

Interpreting liver function test results requires medical expertise, as patterns of abnormal values help identify specific liver conditions. Mildly elevated enzymes might indicate fatty liver disease, while very high levels could suggest acute hepatitis or liver injury. It is important to note that abnormal results do not always mean serious liver disease, as temporary elevations can occur due to medications, recent alcohol consumption, or other factors. If your liver function tests show abnormal results, your healthcare provider may recommend additional testing, imaging studies, or a referral to a liver specialist to determine the underlying cause and appropriate treatment.
