Creatine kinase, also known as CK or creatine phosphokinase (CPK), is an enzyme found primarily in the heart, brain, and skeletal muscles. This enzyme plays a crucial role in energy production within cells. When muscle tissue or other tissues containing CK are damaged, the enzyme is released into the bloodstream. A creatine kinase blood test measures the amount of this enzyme in your blood and helps healthcare providers assess muscle damage or other conditions affecting these tissues.
The CK test is commonly ordered when someone experiences symptoms that suggest muscle damage, such as muscle weakness, muscle pain, or dark-colored urine. It is particularly useful in diagnosing heart attacks, as elevated CK levels can indicate damage to the heart muscle. The test also helps monitor muscle disorders like muscular dystrophy, myositis, and rhabdomyolysis, a serious condition where damaged muscle tissue releases proteins and electrolytes into the blood. Additionally, doctors may order this test to evaluate the effects of certain medications that can cause muscle damage as a side effect, such as statins used to lower cholesterol.
There are three main types of CK found in different parts of the body: CK-MM primarily found in skeletal muscles, CK-MB predominantly in the heart muscle, and CK-BB mainly in the brain. When a CK test shows elevated levels, additional testing to determine which type of CK is elevated can help pinpoint the source of tissue damage. For example, elevated CK-MB levels suggest heart muscle damage, while elevated CK-MM levels typically indicate skeletal muscle injury.
Normal CK levels can vary depending on factors such as age, sex, race, and muscle mass. Generally, men tend to have higher CK levels than women because they typically have more muscle mass. Athletes and people who engage in intense physical activity may also have higher baseline CK levels. Elevated CK levels can result from various causes including strenuous exercise, muscle trauma, injections, surgery, prolonged immobilization, seizures, certain medications, infections, and inflammatory muscle diseases. Very high CK levels usually indicate significant muscle damage and require prompt medical attention.
