TSH is produced by the pituitary gland in your brain and signals the thyroid to produce hormones. When thyroid hormone levels are low, TSH increases to stimulate more production. Conversely, when thyroid hormones are high, TSH decreases. Free T4 and free T3 are the actual hormones produced by the thyroid gland that circulate in your blood and affect your body's cells. The "free" portion refers to the hormone that is not bound to proteins and is available for your body to use.

Your doctor may order thyroid function tests if you have symptoms suggesting thyroid problems, such as unexplained weight changes, fatigue, changes in heart rate, sensitivity to temperature, hair loss, or mood changes. These tests are also used to monitor people already diagnosed with thyroid disorders and those taking thyroid medication. Pregnant women and newborns may also need thyroid testing, as thyroid hormones are crucial for normal development.

The test requires a simple blood sample, usually drawn from a vein in your arm. Most thyroid function tests do not require fasting, though your healthcare provider will give you specific instructions if needed. Results typically show whether your thyroid is functioning normally, overactive (hyperthyroidism), or underactive (hypothyroidism). Your healthcare provider will interpret the results based on your individual situation, symptoms, medical history, and other factors to determine if treatment is necessary.
