Vitamin B12, also known as cobalamin, is an essential nutrient that plays a crucial role in maintaining healthy nerve cells, producing DNA, and forming red blood cells. The body cannot produce vitamin B12 on its own, so it must be obtained through diet or supplements. Testing for vitamin B12 levels is important because deficiency can lead to serious health problems, including anemia, neurological disorders, fatigue, and cognitive impairment. A vitamin B12 blood test measures the amount of this vitamin circulating in your bloodstream to determine if you have adequate, deficient, or excessive levels.
Healthcare providers typically order vitamin B12 testing when patients exhibit symptoms suggestive of deficiency, such as unexplained fatigue, weakness, numbness or tingling in the hands and feet, difficulty walking, memory problems, or mood changes. The test is also commonly recommended for people at higher risk of deficiency, including older adults, vegetarians and vegans, individuals with digestive disorders like Crohn's disease or celiac disease, those who have undergone certain gastrointestinal surgeries, and people taking medications that interfere with B12 absorption such as metformin or proton pump inhibitors.
The vitamin B12 blood test is a simple procedure that requires a standard blood draw from a vein, usually in your arm. Most laboratories do not require fasting before the test, though some healthcare providers may request it. The blood sample is then analyzed to measure the concentration of vitamin B12 in your serum. Normal ranges can vary slightly between laboratories, but generally, levels between 200 and 900 picograms per milliliter are considered normal. Levels below 200 picograms per milliliter typically indicate deficiency, while levels between 200 and 300 may suggest borderline deficiency requiring further evaluation.
In some cases, additional tests may be ordered alongside the vitamin B12 test to provide a more complete picture of your status. These can include methylmalonic acid and homocysteine levels, which become elevated when B12 is deficient, as well as a complete blood count to check for anemia. If results indicate a deficiency, your healthcare provider will determine the underlying cause and recommend appropriate treatment, which may include dietary changes, oral supplements, or vitamin B12 injections depending on the severity and cause of the deficiency.
