Hepatitis B is a viral infection that affects the liver and can cause both acute and chronic disease. The virus is transmitted through contact with infected blood or other body fluids. Understanding the various hepatitis B tests available helps healthcare providers and patients make informed decisions about screening and management.

Several blood tests are used to detect hepatitis B virus infection. The hepatitis B surface antigen test, known as HBsAg, is typically the first test performed to screen for active infection. If this test is positive, it indicates that the person is currently infected with the virus and can transmit it to others. The hepatitis B surface antibody test, or anti-HBs, detects antibodies produced either after successful vaccination or recovery from infection, indicating immunity to the virus. The hepatitis B core antibody test, or anti-HBc, helps distinguish between acute, chronic, and past infections. This test may show positive results in people who have been infected, even if they have recovered.

Additional tests help assess the severity of infection and guide treatment decisions. The hepatitis B e-antigen test, or HBeAg, indicates active viral replication and high infectivity when positive. The corresponding antibody test, anti-HBe, typically appears when viral replication decreases. Hepatitis B viral load testing measures the amount of virus in the blood and helps monitor treatment effectiveness. Liver function tests evaluate how well the liver is working by measuring enzymes and proteins in the blood. Elevated liver enzymes may indicate liver inflammation or damage caused by the virus.

Healthcare providers recommend hepatitis B testing for various groups, including pregnant women, people born in regions with high hepatitis B prevalence, individuals with elevated liver enzymes, people who inject drugs, and those with multiple sexual partners. Regular monitoring through laboratory tests is essential for people with chronic hepatitis B to detect any changes in liver function and assess the need for antiviral therapy. Early detection and appropriate management can prevent serious complications such as cirrhosis and liver cancer.
