Hemoglobin A1c, commonly referred to as HbA1c or glycated hemoglobin, is a crucial laboratory test used to monitor blood sugar control over time. This test measures the percentage of hemoglobin proteins in red blood cells that have glucose attached to them. Because red blood cells live for approximately three months, the HbA1c test provides an average picture of blood glucose levels over the past two to three months. This makes it particularly valuable for assessing long-term blood sugar management rather than just a snapshot of glucose levels at one moment in time.
The HbA1c test is primarily used to diagnose and monitor diabetes mellitus. For diagnosis, an HbA1c level of 6.5 percent or higher on two separate occasions indicates diabetes, while levels between 5.7 and 6.4 percent suggest prediabetes, a condition where blood sugar is elevated but not yet high enough to be classified as diabetes. Normal HbA1c levels are generally below 5.7 percent. For people already diagnosed with diabetes, the test helps healthcare providers evaluate how well treatment plans are working and whether adjustments to medication, diet, or lifestyle are needed.
Regular HbA1c testing is recommended for individuals with diabetes, typically every three to six months depending on how well their condition is controlled. The test requires a simple blood sample and does not require fasting, making it convenient for patients. Unlike daily blood glucose monitoring that can fluctuate based on recent meals, stress, or activity, HbA1c provides a more stable and reliable indicator of overall glycemic control. This information helps predict the risk of developing diabetes-related complications such as kidney disease, nerve damage, and cardiovascular problems.
Several factors can affect HbA1c results and should be considered when interpreting the test. Conditions that affect red blood cell turnover, such as anemia, recent blood transfusions, or certain genetic variants of hemoglobin, may lead to falsely high or low readings. Additionally, pregnancy, kidney disease, and certain medications can influence results. Healthcare providers take these factors into account when using HbA1c values to make treatment decisions. For most people with diabetes, the goal is to maintain an HbA1c level below 7 percent, though individual targets may vary based on age, overall health, and other medical conditions.
