Urobilinogen is a substance produced during the normal breakdown of red blood cells in the body. When red blood cells reach the end of their lifespan, they are broken down in the spleen and liver. Some of this urobilinogen is reabsorbed into the bloodstream and filtered by the kidneys, which is why small amounts normally appear in urine.
A urobilinogen urine test measures the amount of urobilinogen present in a urine sample. This test is often part of a routine urinalysis, which is a common screening tool used to detect various health conditions. The test can help identify problems with liver function, bile duct obstruction, or excessive breakdown of red blood cells. Normal urine contains small amounts of urobilinogen, typically ranging from 0.1 to 1.0 milligrams per deciliter or 0.5 to 4.0 milligrams per 24 hours, though reference ranges may vary slightly between laboratories.
Elevated urobilinogen levels in urine may indicate several conditions. Liver diseases such as hepatitis, cirrhosis, or liver damage can cause increased urobilinogen because the liver cannot properly process bilirubin. Hemolytic anemia, a condition where red blood cells are destroyed faster than normal, also leads to higher urobilinogen levels because more bilirubin is being produced. Heart failure and certain infections can similarly cause elevated readings. Conversely, decreased or absent urobilinogen in urine may suggest a complete blockage of the bile ducts, preventing bilirubin from reaching the intestines, or it may indicate that intestinal bacteria levels are reduced due to antibiotic use.
The urobilinogen test is typically performed using a simple urine dipstick or as part of a comprehensive urinalysis. The test requires a fresh urine sample, as urobilinogen breaks down when exposed to light and air. Healthcare providers often use this test alongside other liver function tests and complete blood counts to get a complete picture of liver health and red blood cell function. If abnormal urobilinogen levels are detected, additional testing may be necessary to determine the underlying cause and appropriate treatment.
