Hemoglobin A1c, commonly abbreviated as HbA1c or A1c, is a crucial blood test used to assess long-term blood sugar control in people with diabetes and to diagnose diabetes or prediabetes. This test measures the percentage of hemoglobin proteins in red blood cells that have glucose attached to them. Since red blood cells live for approximately three months, the A1c test provides an average picture of blood sugar levels over the past two to three months, making it more informative than daily blood glucose measurements that only show a snapshot in time.

The test works by measuring glycated hemoglobin, which forms when glucose in the bloodstream binds to hemoglobin. The higher the glucose levels in the blood over time, the more hemoglobin becomes glycated. Results are expressed as a percentage: a normal A1c level is below 5.7 percent, prediabetes is indicated by levels between 5.7 and 6.4 percent, and diabetes is diagnosed when the A1c is 6.5 percent or higher on two separate tests. For people already diagnosed with diabetes, the American Diabetes Association generally recommends a target A1c of less than 7 percent, though individual targets may vary based on age, health conditions, and other factors.

Healthcare providers typically recommend A1c testing at least twice a year for people with diabetes who are meeting treatment goals and have stable blood sugar control. Those who have changed their treatment plan or are not meeting their goals may need testing more frequently, such as every three months. The test requires a simple blood draw and does not require fasting, making it convenient for patients. However, certain conditions can affect the accuracy of A1c results, including anemia, kidney disease, liver disease, or certain blood disorders that affect red blood cell lifespan.

Understanding your A1c level is important because elevated blood sugar over time can lead to serious complications affecting the eyes, kidneys, nerves, and cardiovascular system. By monitoring A1c levels regularly, people with diabetes and their healthcare providers can determine whether current treatment strategies are effective or if adjustments are needed. This test serves as a vital tool in managing diabetes and reducing the risk of long-term complications associated with poorly controlled blood sugar levels.
