The body cannot produce vitamin B12 on its own, so it must be obtained through diet or supplements. Common dietary sources include meat, fish, poultry, eggs, and dairy products. People who follow vegetarian or vegan diets may be at higher risk of deficiency and might need to consume fortified foods or take supplements.
A vitamin B12 blood test measures the amount of this vitamin circulating in the bloodstream. Healthcare providers typically order this test when they suspect a deficiency or want to monitor levels in patients at risk. Symptoms of vitamin B12 deficiency can include fatigue, weakness, constipation, loss of appetite, weight loss, numbness or tingling in the hands and feet, difficulty maintaining balance, depression, confusion, poor memory, and soreness of the mouth or tongue. Because these symptoms can develop gradually and may be similar to other conditions, laboratory testing is essential for accurate diagnosis.
The test is performed using a simple blood sample, usually drawn from a vein in the arm. Patients may be asked to fast for several hours before the test, although requirements can vary depending on the laboratory and other tests being performed simultaneously. Normal vitamin B12 levels typically range from 200 to 900 picograms per milliliter, though reference ranges may differ slightly between laboratories. Low levels indicate a deficiency that may require treatment with supplements or injections, while extremely high levels are less common but can occur with certain medical conditions or excessive supplementation.
Several factors can affect vitamin B12 levels in the body. Older adults are at increased risk of deficiency due to decreased stomach acid production, which is necessary for vitamin B12 absorption. People with certain digestive disorders such as Crohn's disease, celiac disease, or those who have had gastric surgery may also have difficulty absorbing the vitamin.
