Cytomegalovirus, commonly known as CMV, is a widespread virus that belongs to the herpes virus family. Most healthy individuals who contract CMV experience no noticeable symptoms and may not even realize they have been infected. The virus remains dormant in the body after initial infection and can reactivate under certain circumstances, particularly when the immune system becomes weakened. While CMV typically poses little threat to people with healthy immune systems, it can cause serious complications in newborns, pregnant women, and individuals with compromised immunity.

Laboratory testing for CMV is essential in several clinical situations. The most common tests include CMV antibody tests that detect IgM and IgG antibodies in the blood. IgM antibodies appear first during an active or recent infection, while IgG antibodies develop later and remain in the body for life, indicating past exposure to the virus. Healthcare providers may also order CMV DNA tests, which use polymerase chain reaction technology to detect the actual genetic material of the virus in blood, urine, or other body fluids. These molecular tests are particularly useful for monitoring viral load in immunocompromised patients or diagnosing congenital CMV infection in newborns.

Testing for CMV is particularly important during pregnancy, as the virus can be transmitted from mother to baby and potentially cause birth defects or developmental problems. Pregnant women who have never been exposed to CMV are at higher risk of transmitting the infection to their unborn child if they contract it during pregnancy. Regular screening helps identify women who may need closer monitoring. Additionally, CMV testing is routinely performed before and after organ transplantation, as transplant recipients taking immunosuppressive medications are at increased risk for CMV reactivation or new infection.

Results from CMV tests help healthcare providers determine whether someone has an active infection, a past infection, or has never been exposed to the virus. Negative IgM and IgG results suggest no previous exposure, while positive IgG with negative IgM typically indicates past infection. Positive IgM results may suggest recent or active infection, though additional testing is often needed for confirmation. For immunocompromised patients, quantitative CMV DNA tests measure the amount of virus present, helping doctors decide when to start or stop antiviral treatment. Understanding these test results is crucial for appropriate medical management and preventing complications in vulnerable populations.
