Serum creatinine is a waste product that results from the normal breakdown of muscle tissue in the body. It is produced at a relatively constant rate and is filtered out of the blood by the kidneys. Measuring the level of creatinine in the blood is one of the most common laboratory tests used to evaluate kidney function. When the kidneys are working properly, they efficiently remove creatinine from the bloodstream and excrete it in the urine. However, when kidney function declines, creatinine begins to accumulate in the blood, leading to elevated serum creatinine levels.

A serum creatinine test is typically ordered as part of a comprehensive metabolic panel or basic metabolic panel during routine health checkups. It may also be requested when a healthcare provider suspects kidney disease or wants to monitor existing kidney conditions. The test is particularly important for people with diabetes, high blood pressure, or other conditions that can affect kidney function. It is also used to monitor patients taking medications that may impact the kidneys and to assess kidney function before certain medical procedures or imaging studies that use contrast dye.

The normal range for serum creatinine varies depending on several factors including age, sex, muscle mass, and the laboratory performing the test. Generally, normal values range from approximately 0.6 to 1.2 milligrams per deciliter for adult men and 0.5 to 1.1 milligrams per deciliter for adult women. Men typically have higher creatinine levels than women because they usually have more muscle mass. Elevated creatinine levels may indicate impaired kidney function, dehydration, muscle disorders, or certain medications. Low creatinine levels are less common but may occur in people with decreased muscle mass, malnutrition, or certain chronic diseases.

The serum creatinine test is often used together with other measurements to calculate the glomerular filtration rate, which provides a more accurate assessment of how well the kidneys are filtering waste from the blood. Blood urea nitrogen is another test frequently performed alongside serum creatinine to evaluate kidney function. The test requires a simple blood draw from a vein, usually in the arm, and no special preparation is typically needed, though some laboratories may request fasting or temporary discontinuation of certain medications. Results are usually available within a day or two, and any abnormal findings should be discussed with a healthcare provider who can interpret them in the context of the individual's overall health status and medical history.
