Hepatitis B is a viral infection that affects the liver and can cause both acute and chronic disease. The virus is transmitted through contact with infected blood or other body fluids. Laboratory testing plays a crucial role in diagnosing hepatitis B infection, determining the stage of disease, and monitoring treatment response. Understanding the different hepatitis B tests and their meanings helps patients and healthcare providers make informed decisions about management and care.
The hepatitis B surface antigen test, known as HBsAg, is typically the first test performed when hepatitis B infection is suspected. A positive HBsAg result indicates active infection with the hepatitis B virus, whether acute or chronic. If this test remains positive for more than six months, it suggests chronic infection. The hepatitis B surface antibody test, or anti-HBs, detects antibodies produced in response to vaccination or recovery from infection. A positive anti-HBs result generally indicates immunity to hepatitis B, either from successful vaccination or from clearing a previous infection.
Additional hepatitis B tests provide more detailed information about the infection status. The hepatitis B core antibody test comes in two forms: IgM anti-HBc, which indicates recent or acute infection, and total anti-HBc, which shows current or past infection. The hepatitis B e antigen test, or HBeAg, helps determine how contagious an infected person is and whether the virus is actively replicating. When HBeAg is positive, it typically indicates high viral activity and greater infectiousness. The corresponding antibody test, anti-HBe, usually appears when viral replication decreases.
The hepatitis B viral load test, also called HBV DNA quantification, measures the amount of virus present in the blood. This test is essential for monitoring chronic hepatitis B infection and determining whether antiviral treatment is needed. Higher viral loads generally indicate more active disease and greater risk of liver damage. Liver function tests, including measurements of liver enzymes such as ALT and AST, are also important in evaluating hepatitis B infection, as elevated levels may indicate liver inflammation or damage. Regular monitoring through these laboratory tests helps healthcare providers assess disease progression and treatment effectiveness, ensuring optimal management of hepatitis B infection.
