Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is an important blood test used to measure average blood sugar levels over the past two to three months. This test provides valuable information about how well diabetes is being managed or can help diagnose diabetes and prediabetes. Unlike daily blood glucose tests that show sugar levels at a specific moment, HbA1c gives a broader picture of blood sugar control over time. When glucose in the bloodstream attaches to hemoglobin in red blood cells, it forms glycated hemoglobin. Since red blood cells live for about three months, measuring the percentage of glycated hemoglobin reflects average glucose levels during that period.
The HbA1c test is typically performed using a simple blood sample drawn from a vein in your arm, though some facilities may use a fingerstick sample. No special preparation is needed before the test, meaning you can eat and drink normally beforehand. This convenience makes it easier to schedule compared to fasting blood glucose tests. Results are reported as a percentage, with normal levels generally below 5.7 percent. A result between 5.7 and 6.4 percent indicates prediabetes, while a level of 6.5 percent or higher on two separate tests suggests diabetes. For people already diagnosed with diabetes, the target HbA1c level is usually below 7 percent, though individual goals may vary based on age, health conditions, and other factors.
Healthcare providers recommend HbA1c testing at different frequencies depending on individual circumstances. People without diabetes who are at risk may be tested annually as part of routine screening. Those with prediabetes might be tested once or twice yearly to monitor their condition. For individuals with diabetes, testing frequency depends on how well blood sugar is controlled and the type of treatment being used. People meeting treatment goals may need testing only twice a year, while those adjusting medications or not meeting targets may require testing every three months.
Several factors can affect HbA1c test results and should be considered when interpreting the values. Certain medical conditions affecting red blood cells, such as anemia, recent blood loss, or blood transfusions, can lead to inaccurate results. Some hemoglobin variants, which are more common in people of African, Mediterranean, or Southeast Asian descent, may also interfere with certain testing methods. Pregnancy, kidney disease, and liver disease can influence results as well.
