Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is a crucial blood test used to measure average blood sugar levels over the past two to three months. This test provides valuable information about long-term glucose control and is essential for diagnosing and monitoring diabetes. Unlike regular blood glucose tests that show sugar levels at a specific moment, HbA1c reflects how well blood sugar has been controlled over an extended period. The test measures the percentage of hemoglobin proteins in red blood cells that have glucose attached to them. Since red blood cells live for about three months, the HbA1c test gives healthcare providers a comprehensive picture of average blood glucose levels during that time frame.

For people without diabetes, a normal HbA1c level is typically below 5.7 percent. Levels between 5.7 and 6.4 percent indicate prediabetes, a condition where blood sugar is higher than normal but not high enough to be classified as diabetes. An HbA1c level of 6.5 percent or higher on two separate tests indicates diabetes. For individuals already diagnosed with diabetes, the target HbA1c level is generally below 7 percent, though this target may vary based on individual circumstances, age, and other health conditions. Healthcare providers use these measurements to adjust treatment plans and medications to help patients maintain optimal blood sugar control.

The HbA1c test is recommended at least twice a year for people with diabetes who are meeting their treatment goals and have stable blood sugar levels. Those who have recently changed their diabetes treatment or are not meeting their blood sugar targets may need to be tested more frequently, typically every three months. The test is also used as a screening tool for people at risk of developing diabetes, including those with a family history of the condition, obesity, or other risk factors. No special preparation is needed for this test, and it can be performed at any time of day without fasting.

Several factors can affect HbA1c test results and should be considered when interpreting the values. Conditions that affect red blood cell turnover, such as anemia, recent blood loss, or blood transfusions, can influence results. Certain hemoglobin variants found in some ethnic populations may also interfere with test accuracy. Additionally, pregnancy, kidney disease, and liver disease can impact HbA1c levels. Understanding these limitations helps healthcare providers choose the most appropriate testing methods and interpret results accurately for each individual patient.
