Procalcitonin Testing: Understanding This Important Infection Marker
Procalcitonin is a substance produced by the body in response to bacterial infections and sepsis. When the body detects a serious bacterial infection, cells throughout various tissues begin to release procalcitonin into the bloodstream. Measuring procalcitonin levels through a blood test helps healthcare providers distinguish between bacterial and viral infections, guide antibiotic therapy decisions, and monitor the severity of infections. Unlike other infection markers, procalcitonin levels rise quickly within a few hours of bacterial infection and remain elevated as long as the infection persists.
The procalcitonin test is primarily used in hospital settings to evaluate patients with suspected sepsis or severe bacterial infections. It helps doctors determine whether antibiotics are necessary and when they can be safely discontinued. This is particularly valuable because unnecessary antibiotic use contributes to antibiotic resistance, a growing global health concern. The test is also useful for monitoring patients with known infections to assess whether treatment is working effectively. In cases of respiratory infections, procalcitonin testing can help differentiate between bacterial pneumonia requiring antibiotics and viral infections that do not.
Normal procalcitonin levels in healthy individuals are very low, typically below 0.05 nanograms per milliliter. Levels between 0.05 and 0.5 suggest a low risk of severe bacterial infection or sepsis. Moderately elevated levels between 0.5 and 2.0 indicate a moderate risk and suggest possible bacterial infection. Levels above 2.0 strongly suggest severe bacterial infection or sepsis, while levels exceeding 10.0 indicate a high likelihood of severe sepsis or septic shock. However, these values should always be interpreted by healthcare providers in the context of clinical symptoms and other diagnostic findings.
Several conditions besides bacterial infections can affect procalcitonin levels. Severe trauma, major surgery, prolonged severe shock, and certain types of cancer can elevate procalcitonin even without bacterial infection. Newborns may have temporarily elevated levels during the first few days of life. Some autoimmune conditions and kidney disease can also influence test results. For this reason, procalcitonin testing is most useful when combined with other clinical assessments, physical examinations, and laboratory tests to provide a complete picture of a patient's condition and guide appropriate treatment decisions.
