Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is a laboratory test that measures the average blood sugar level over the past two to three months. This test is primarily used to diagnose and monitor diabetes mellitus. Unlike daily blood glucose tests that provide a snapshot of sugar levels at a specific moment, HbA1c gives healthcare providers a broader picture of how well blood sugar has been controlled over time. The test measures the percentage of hemoglobin proteins in red blood cells that have glucose attached to them. Since red blood cells live for approximately three months, the test reflects the average glucose exposure during that period.

The HbA1c test is performed using a simple blood sample, which can be drawn at any time of day without requiring fasting. This convenience makes it easier for patients compared to traditional glucose tolerance tests. For people without diabetes, normal HbA1c levels are typically below 5.7 percent. A result between 5.7 and 6.4 percent indicates prediabetes, a condition where blood sugar levels are higher than normal but not yet high enough to be classified as diabetes. An HbA1c level of 6.5 percent or higher on two separate tests indicates diabetes. For individuals already diagnosed with diabetes, the target HbA1c level is usually below 7 percent, though this goal may vary based on individual circumstances and medical history.

Regular HbA1c testing is essential for people with diabetes to assess how well their treatment plan is working. The test helps determine whether current medications, diet, and lifestyle modifications are effectively controlling blood sugar levels. Most healthcare providers recommend that people with well-controlled diabetes have their HbA1c tested at least twice a year, while those whose treatment has changed or who are not meeting their goals may need testing more frequently, typically every three months.

Several factors can affect HbA1c results and should be considered when interpreting the test. Conditions that affect red blood cell turnover, such as anemia, recent blood loss, blood transfusions, or certain genetic hemoglobin variants, may lead to inaccurate results. Additionally, kidney disease, liver disease, and certain medications can influence HbA1c levels. It is important to discuss any medical conditions or medications with your healthcare provider, as they may recommend alternative testing methods if factors are present that could interfere with accurate HbA1c measurement. Despite these limitations, HbA1c remains one of the most valuable tools for diabetes management and screening.
