Coagulation tests are essential laboratory examinations that evaluate how well your blood clots. Healthcare providers commonly order coagulation tests before surgery, to monitor patients taking blood-thinning medications, or when investigating bleeding or clotting disorders. The most frequently performed coagulation tests include prothrombin time, activated partial thromboplastin time, international normalized ratio, and platelet count.

Prothrombin time measures how long it takes blood plasma to clot and evaluates the extrinsic and common pathways of the coagulation cascade. This test is particularly important for monitoring patients taking warfarin and other anticoagulant medications. The international normalized ratio is a standardized version of the prothrombin time that allows for consistent results across different laboratories. Activated partial thromboplastin time assesses the intrinsic and common pathways of coagulation and is used to monitor heparin therapy and screen for clotting factor deficiencies.

Additional coagulation tests may include fibrinogen level, D-dimer, bleeding time, and specific clotting factor assays. Fibrinogen is a protein essential for clot formation, and low levels can indicate liver disease or disseminated intravascular coagulation. D-dimer tests detect protein fragments produced when blood clots dissolve and are useful in diagnosing conditions like deep vein thrombosis or pulmonary embolism. Platelet function tests evaluate how well platelets work together to form clots, which is important for diagnosing bleeding disorders.

Preparation for coagulation tests typically involves fasting for several hours before the blood draw, though specific requirements vary depending on which tests are ordered. Patients should inform their healthcare provider about all medications they are taking, especially blood thinners, aspirin, and nonsteroidal anti-inflammatory drugs, as these can affect test results. The blood sample is usually collected from a vein in the arm using a standard blood draw procedure. Results help doctors diagnose bleeding disorders, monitor anticoagulation therapy, assess liver function, and evaluate the risk of excessive bleeding during surgical procedures.
